Optima Health Community Care is a Virginia Medicaid managed care program health plan. It is for people who may need help with daily tasks like dressing and bathing, or have a physical disability, long-term disease, addiction or mental illness. You will get a care coordinator to connect you with trusted doctors and services like access to healthy food, housing, and transportation. Apply at any time of year.
