Optima Community Complete (HMO SNP) is a Dual Special Needs Plan (D-SNP) that offers members a more specialized level of care for highly sensitive needs. If you receive Medicaid and are also eligible for Medicare, a D-SNP plan may be right for you. This coverage is a type of Medicare Advantage plan that increases the level of benefits that you would get from Original Medicare or Medicaid alone. You’ll still get the same great benefits from your Medicaid coverage, plus enhanced care coordination and management of your medical needs.

Along with your medical providers, you will also have a Care Coordinator to help you stay healthy and follow your doctor’s recommendations. Your Care Coordinator will help you get the right care and the right information you need to keep you feeling your best.
