Glossary Terms

An individual who is eligible for both Medicare and Medicaid benefits.

The Federal health insurance program for people 65 years of age or older, some people under age 65 with disabilities, and people with End-Stage Renal Disease (generally those with permanent kidney failure who need dialysis or a kidney transplant).

A jointly funded, Federal-State health insurance program that helps with medical costs for people with low income and limited resources. It covers children, the aged, blind, and/or disabled and other people who are eligible to receive federally assisted income maintenance payments.

Managed Long Term Services and Supports (MLTSS) refers to the delivery of long-term services and supports through Virginia Medicaid's managed care program. MLTSS is designed to expand home and community-based services, promote community inclusion and ensure quality and efficiency. Optima Health offers this program through the product Optima Health Community Care.

CCC Plus:
Commonwealth Coordinated Care Plus (CCC Plus) is a new statewide Medicaid managed care program that serves individuals with complex care needs through an integrated delivery model across the full continuum of care.  Care management is at the heart of the CCC Plus high-touch, person-centered program design. Optima Health offers this program through the product Optima Health Community Care.
