Accessing your vision benefits is easy:
- Select a participating provider. If you need assistance, call 1-888-610-2268.
- Schedule an appointment or visit the participating provider.
- Share your member ID card information. The optical provider will verify your eligibility, plan benefits, and any fees that might apply.

Special Discounts
Contact Lens Replacement
If you “run out”, lose, or tear your contacts, call eyemedcontacts.com at 1-800-508-1399 with your prescription to request replacement lens. There is a $2.95 administration fee for the phone order service. There is no administration fee if lenses are ordered via the internet at www.eyemedcontacts.com. A Service Representative will assist you with pricing and ordering. Upon release of your contact lens prescription by your eye care provider, your lenses will ship via UPS Standard delivery and should arrive approximately within 2-4 days. Faster shipping options are available at an additional cost.
Laser Vision Correction
You are eligible to receive 15% off "Standard" or "Usual & Customary" price of LASIK or PRK surgery from a U.S. Laser Network provider (or 5% off any promotional price if lower). Simply call 1-877-5LASER6 (1-877-552-7376) to begin the process and locate a provider nearest you. After scheduling your surgery, be sure to take your member ID card and confirmation number with you to the appointment.
