Well child exams are a very important part of children’s health care.
It is important that your patients visit you every year for a Well Child or Adolescent Well Child visit, even if they haven't been sick. These visits give you the opportunity to examine the child for overall health, growth and development.
Optima Health has developed Well Child and Adolescent Well Child forms to help improve the quality and documentation of preventive and Early Periodic Screening, Diagnosis, and Treatment (EPSDT); to address Health Plan Employer Data and Information Set (HEDIS) measures; and to provide continuity in data collection.
Directions for using the forms in your practice:
- Use the form appropriate for the patient's age in place of your current chart note.
- Please complete the form. You may use the back of the top form sheet if you need additional writing or documentation space.
- Place form in chart.
