As of July 1, 2008 Optima Health began to issue new member identification cards based on the member’s renewal date. The new ID cards have been re-designed to make them easier to read, for both members and providers.

Please note that the primary care physician (PCP) name is no longer shown on the ID card, because most plans no longer require referrals.  For all plans that require referrals, this is indicated on the card.

To determine a member’s PCP, you may utilize Interactive Voice Recognition (IVR), or the Provider Relations Office.
