Web based access to Optima Health medical plan information including:
- Member eligibility
- Referral submission and referral status
- Detailed claim status
- PCP membership reports
- Access to Optima Health plan documents and manuals, such as:
- Clinical Guidelines
- Drug Authorization Forms
- EDI Set-Up Fact Sheet
- Employer Choices Manual
- Obstetrics Certification Form
- Oxygen Ordering Form
- Physician Order for Diabetes Supplies
- Provider Reconsideration Form

Who can sign up for optimahealth.com's Provider Connection?
Any healthcare providers or staff (i.e., physicians, clinicians, office staff) that may require access to member information such as eligibility, claim status, referral status, and/or referral creation.
Please note that this is currently not available for our mental health providers.
How do I get started?
It's easy. Simply:
- Verify that your hardware meets the minimum system requirements.
- Internet access. For optimum performance, high-speed internet access is desired (DSL, cable or network).
- All authorized users must agree to Sentara's Statement of Responsibility and Confidentiality.
- Complete the online Provider Connection Optima Health Enrollment form.
- You will be contacted by Network Management with your sign on ID and password.
Can I register for Provider Connection online?
Yes. Simply complete the Provider Connection Optima Health Enrollment form.
