Did you know that Optima Health now accepts the CAQH (Coalition for Affordable Quality Healthcare) application for initial credentialing and re-credentialing?
For initial credentialing, simply contact your Network Educator at 1-877- 865-9075 or 757-552-7085 to let him/her know you will be using a CAQH application. At that time, an Optima Health release form will be sent. Once the signed release form has been completed and received by Optima Health, the credentialing process can begin.
Don't forget that you must also contact CAQH to give permission to release the provider's application to Optima Health. Once the release form is received and the CAQH application is retrieved, the documents will be forwarded to the Credentialing Department for processing.
If you are currently participating with Optima Health all you need to do is notify our Credentialing Department that you have a CAQH application on file and have given CAQH permission to release your application. Then, at the time of re-credentialing, you will only need to complete an Optima Health release form.
If you have not yet completed the CAQH application you may continue to use the Optima Health credentialing and re-credentialing application.
