Manage Your Optima Design Account with Choice Care Card™

 

Currently, Optima Design members manage their account through the Choice Care Card™ Web site. To do so, Optima Design members must create an account at, and log in to, the Choice Care Card™ Web site. The following instructions will help you create your account and get logged in.

 

You will first need to access the Choice Care Card™ Web site.*

 

Once at the Choice Care Card™ Web site, you will see the following log in screen. You will either need to create an account (if you haven't already done so) or log in.

If you chose "Create Account" on the Choice Care Card™ log in page, you will need to refer to your Optima Health Member ID card to enter your "Member Number" and "Group Number".

 

Please see the graphic below to locate your "Member Number" and "Group Number" on your Optima Health Member ID.

After referring to your Optima Health Member ID card, enter the information in the online form on the Choice Care Card™ Web site.

 

Please note: as indicated on the graphic below, you will need to enter your Optima Health "Member Number" in the field labeled "Employee ID" on the Choice Care Card™ Web site; you will need to enter your Optima Health "Group Number" in the field labeled "Employer ID" on the Choice Care Card™ Web site.

After entering the information indicated in the above graphic, you will be required to create a "New User ID", "Password" and "Security Word" and enter your "Birth City."
