Definitions

 

Specialty - The physician reports to Optima the specialty they practice at the time of initial credentialing and is verified by Optima during the credentialing process. If a change occurs a change form is filled out and the change is made in the Provider Relations Database otherwise all information is re-verified every three years during the re-credentialing process.

 

Hospital Affiliations - A physician is required to report to Optima all hospitals they have privileges with as well as the type of privileges at the time of initial credentialing. Optima verifies the information with participating hospitals. If a change occurs a change form is filled out and the change is made otherwise all information is re-verified every three years during the re-credentialing process.

 

Medical Group Affiliations - A physician is required to report to Optima all practices they participate with as well as locations on the initial credentialing form. This is self reported and not verified by Optima. If a change occurs a change form is filled out and the change is made in the Provider Relations Database.

 

Languages spoken - A physician is has the option to provide languages spoken on the initial credentialing application, it is not required. Optima does not verify the languages listed on the application. If a change occurs a change form is filled out and the change is made in the Provider Relations Database or updated by the physician every three years during the re-credentialing process.
