If you are a current Optima Medicare member and you need to change your existing Medicare Plan, please review the information on this page. If you would like to speak to a representative, please call 1-800-927-6048 (TTY/TTD 1-800-225-7784).
If you need to change plans for 2010, you have several options. You can change by phone, download and mail an application, request an enrollment kit, or change plans online at the Medicare Web site. These options are described in detail below.
To change your plan by phone, simply call Optima Health at 1-800-927-6048 (TTY/TTD 1-800-225-7784).
You may download, complete, sign and date an enrollment form. The form is a writable PDF document, so you can fill it out on your computer then print it, or print it and fill it out later. Once you've completed the form, please mail it to:
