The Release of Information Designated Agent Form (ROI) is a form that is needed by Optima Health if, and only if, you wish to allow someone else to obtain and/or change your health information (such as access to claims information, ability to change your Primary Care Physician (PCP) or update your contact information.)
Below are answers to some frequently asked questions about the Release of Information Form (ROI).
Q. Why do I need an ROI for my spouse if I am the subscriber?
A. Any member 18 years old or older is considered an adult – which means his/her medical information is confidential and private. Even though you are the policyholder (subscriber), you do not have access to your adult spouse’s medical or claims information. This philosophy also applies to your children who are on the plan that are 18 and older. The adult member will have to complete the ROI and name you as a designated agent if he/she wishes you to have access to their claims and account information.
Q. My spouse has insurance for my child, why do I need an ROI for a minor?
A. If you are the policyholder (subscriber), you can request information for your minor child. If you wish for your spouse to have access to your minor child’s account (such as changing PCPs or reviewing authorization / claims status), you will have to give permission for your spouse to act as a designated agent for the minor child.
Q. Is this something I have to sign?
A. Only if you want to allow someone else to have access to your claims and other policy information, or the ability to change your PCP, phone, address or email. If you do not want or need anyone else to have access to your information at this time, you do not need to return this form.
Q. Does this have to be notarized?
A. No, the form does not have to be notarized; however, you do need to have a witness other than the person you are designating as your agent sign the form.
Q. If I don’t complete this, will I still be able to call in and obtain info regarding my own account?
A. Yes.
Q. Do I need this in case I die and my spouse or family member needs to check on my account?
A. No.
Q. Can anyone else make changes to or view my information?
A. We require a Release of Information/Authorization of Designated Agent form whenever anyone other than the Optima Health member needs to obtain and/or change health information. This form must be signed, witnessed and returned to Optima Health.
