- Select a participating provider. If you need assistance, call 1-888-610-2268.
- Schedule an appointment or visit the participating provider.
- Share your member ID card information. The optical provider will verify your eligibility, plan benefits, and any fees that might apply.

Special Discounts
Contact Lens Replacement
Call Contacts Direct at 1-800-987-5367 with your prescription to replace your contacts. The Contacts Direct Program lets you purchase the same brand contact lenses your doctor prescribed-for less. Your lenses are shipped directly to your home address. Overnight service is available.
Laser Vision Correction
Health plan members are eligible to receive up to a 15 percent discount off the cost of LASIK laser surgery (or five percent off a promotional price if lower). If you are interested in LASIK surgery, call 1-888-705-2020 to select a participating provider.
