Here's how:
- Select a participating provider. You will also receive directions to your participating provider's facility and a Provider Access Certificate that includes the provider's fee schedule.
- Call the participating provider and schedule an appointment. A physician referral is not necessary. There are no annual visit limitations and you may change participating providers at any time.
- Upon arrival, show the participating provider your Optima Health member ID card to verify eligibility. Then pay the discounted fee at the time of your visit. There are no claim forms to file.

If you have a chiropractic medical benefit under the Plan, you may want to use this discount program after you have reached your annual Plan limits, or for services not covered under that benefit.
Do I need a referral or pre-authorization?
No. Whether you are an HMO, POS or PPO member, you do not need to get referrals or pre-authorizations to take advantage of this discount program. Just follow the three-step process of: 1) selecting a participating provider, 2) scheduling an appointment, and 3) receiving and paying for services.
Do I need to submit claim forms? No.
Do I need permission to change participating providers in the middle of a treatment plan? No. You may change providers at any time.
