As of July 1, 2008 Optima Health began to issue new member identification cards based on the member’s renewal date. The new ID cards have been re-designed to make them easier to read, for both members and providers.

Please note that the primary care physician (PCP) name is no longer shown on the ID card, because most plans no longer require referrals.  For all plans that require referrals, this is indicated on the card.

To determine your primary care physician, please call Member Services at the number shown on your ID card (8 a.m. to 7 p.m. EST Monday-Friday) or contact us.
