Please read and follow the instructions included with your test kit carefully.
When completed, simply mail it to Optima Health in the pre-addressed envelope. It is very important that you include your primary care physician’s (PCP) name and phone number, so he/she can receive your test results and follow up with you if needed.
If you have particular factors that increase your risk of colon cancer, your PCP may recommend a different screening test for you. We encourage you to follow his/her advice.
