Adding Your New Addition to the Plan


Need to know when and how to add your newborn or newly adopted child to your Plan?  Read on.

When:

All newborns and adopted children must be added to the Plan within 31 days of birth or placement for adoption.

How:

Submit any documentation for these additions directly to your employer for processing. Failure to provide information requested by the Plan within 30 days from the effective date of coverage may result in your dependent being ineligible for coverage until the next open enrollment period or qualifying event.

Documentation Required:

Biological child with same last name as parent(s):

  • Hospital birth records

Adopted or children with a different last name:

  • Birth certificate and a marriage certificate are required to establish the relationship between the subscriber and the dependent.

Adopted children or court-ordered benefits:

  • Adoption papers or the court order is required.
