Occasionally employees enroll dependents who have a different last name.
Previously, Optima Health required various forms be submitted for proof of dependent status prior to enrollment of the dependent.
Optima Health no longer requires this documentation be provided prior to enrollment of the dependent, but may be requested at a future date.
As a reminder, eligible dependents include:
- The legal spouse of the insured employee.
- Unmarried children under 19 years of age. (Coverage terminates at the end of the month in which the child turns 19 years of age).*
- The insured employee’s unmarried children who are enrolled full time (undergraduates with 12 or more credit hours per semester; graduates with six or more credit hours) in an accredited university or college, and are not employed anywhere on a full-time basis. (Coverage terminates at the end of the month in which the child turns 24 years of age or is no longer a full-time student.)
- The insured employee’s natural children, legally adopted children, or any other children who are supported solely by the employee and who are permanently residing in the insured employee’s household. Stepchildren, whether residing with the member or not, are also eligible.
- Children who are unmarried over age 19 who are unable to care for themselves as a result of mental or physical handicap occurring prior to age 19 who have been continually covered on any plan.
*These are small group and standard age limits for Optima Health plans. Some large group employers may have modified dependent age requirements documented with the plan.
